How Blepharoplasty is Performed

Dr. Pugash uses several different surgical techniques to produce customized results for patients. The right blepharoplasty procedure for you depends on your cosmetic concerns and ultimate goals. Blepharoplasty surgery of the eyelids can produce very pleasing facial rejuvenation, leaving little or no evidence that any operation has been carried out. The goal is a natural, more youthful-looking eyelid with a defined natural crease. You and Dr. Pugash will decide on the technique that is best for you during your initial consultation.

Depending on your goals, Dr. Pugash may perform upper blepharoplasty (upper eyelid surgery) and/or lower blepharoplasty (lower eyelid blepharoplasty). In some cases, ptosis repair may be discussed if droopy upper eyelids are caused by eyelid muscle weakness rather than excess skin alone.

Upper Eyelid Surgery

Upper eyelid surgery (upper eyelid surgery/upper blepharoplasty) addresses droopy eyelids and sagging eyelid skin that hangs over the eyes and fatty deposits that create a bulky lid appearance. This surgical procedure requires an incision along the natural crease of the upper eyelid. A crescent of excess skin (extra skin) is then removed, and underlying fat is reduced (fat removal/remove excess fat). The skin is carefully stitched so that only a minimal scar is created at the natural eyelid crease.

The result of upper eyelid blepharoplasty is a defined eyelid contour with a smooth, youthful appearance to the skin. This procedure is also an excellent option for creating a double fold for Asian patients. It is also chosen by patients who have naturally hooded eyelids, rather than age-related changes.

Lower Eyelid Surgery

Lower eyelid surgery (lower eyelid surgery/lower blepharoplasty) treats fatty deposits under the eyes (bags under the eyes/under eye bags) and drooping lower eyelid skin. The incision is made on the inside of the lower eyelid or just below, at the lower lash line, depending on the patient’s cosmetic goals. Through these incisions, excess fat is removed or the surgeon may reposition fat to smooth contour, and sagging skin is trimmed (remove excess skin). The results of lower eyelid surgery provide a naturally refreshed and well-rested appearance.

Dr. Pugash provides complete care following your eyelid surgery, so you remain comfortable, return quickly to all your usual activities, and obtain the best possible results. During the recovery period, he will ask you to return several times to his office for follow-up visits. At that time, Dr. Pugash will assess your healing progress and answer any questions you might have. Also, he may also evaluate your candidacy for future treatments, such as short scar facelift. Patients are usually presentable within two to two-and-a-half weeks after blepharoplasty. You may experience swelling and bruising during the first week; cold compresses and keeping your head elevated can help. Mild or slight discomfort is common early on.

Rare risks can include blood clots, detached retina, or bleeding from blood vessels; certain medical conditions such as thyroid problems, high blood pressure/blood pressure concerns, circulatory disorders, or other circulatory disorders may affect surgical planning.

Patients can wear eyeglasses immediately after surgery. It’s recommended that patients wait at least two weeks before inserting contact lenses because this can disrupt the incisions. Dr. Pugash will provide you with detailed aftercare information and will let you know when you can resume wearing contacts. Some patients are advised to wear dark sunglasses outdoors during early healing.

Most of the swelling and bruising will be gone after two weeks. While the eyelids appear very much improved at this point, the final results will not be apparent for six to twelve months.
