Silicone breast implants consist of a Silastic (silicone rubber) shell with a cohesive silicone gel filling. These implants can help achieve a remarkable yet natural-looking breast enhancement, and they also may feel more natural when compared to saline implants.

They are generally used in a smooth, round configuration. These implants produce less rippling than saline devices. They can be used behind or in front of the pectoralis muscle. Current cohesive silicone gel devices have been improved to limit leakage of the gel outside the implant. Canadian regulations limit use of cohesive silicone gel devices to patients aged 22 years or older.
It is generally expected that a cohesive silicone gel breast implant will last 15 to 20 years before requiring replacement.
Cohesive silicone gel implants are available with a textured surface. It is possible that the texturing reduces the risk of scar tissue build-up around the implant, also known as capsular contracture.
